Objections to uses and disclosures:

  1. You have the right to object before your medical information can be used.  If you do not object after given the opportunity your medical information may be disclosed to your family members, other relatives and close personal friends, as needed to know if they are involved in caring for you.
  2. Disaster Relief:  We may reveal your medical information to a public or private disaster relief organization assisting with an emergency.

Other rights:

  1. You have the right to look at your information and obtain a copy.  There will be a charge for copies. Requests for copies must be in writing.
  2. Any requests must be in writing to amend the information.
